Because VNA is dedicated to expert patient care, we have, on staff, three clinical specialist registered nurses, each a recognized expert in wound,colostomy and/or IV therapy. Every day, these clinical specialists teach and consult with nurses, make home visits and consult with physicians to assure that our patients receive the most therapeutic and up-to-date care.
24/7 admission capability.
VNA is able to admit and care for patients 24 hours a day, seven days a week. A registered nurse is in the office from 8 a.m. to 10 p.m. seven days a week to answer patient and family questions, advise patients on current treatment, consult with field staff, follow up and be available to patients’ caregivers and physicians.
Experienced physical therapists, occupational therapists and speech pathologists provide evaluations, advice and treatments to our patients, all to assist them in regaining their independence and allowing them to live and function in their home environments.
Medical Social Workers are skilled in evaluating and assisting our patients and their families in accessing resources and accomplishing long-term planning.
VNA professionals are committed to patient safety.
The Visiting Nurse Association team wants each of our patients to be safe in his or her home environment. To accommodate you or your loved one, our registered nurses (RNs) and certified nursing aides (CNAs) make home visits, when needed, to help with IVs, assist patients with transfer to and from bed and offer similar task support.
Because falls in the home are a prevalent concern, VNA’s team of nurses, therapists and paraprofessionals works with patients and families to prevent falls. Our safety program focuses on assessing patient risks and providing appropriate intervention.
NOTE: Patients and families can be comforted by the fact that VNA does a criminal background check on every employee and volunteer who works for VNA.
We work closely with your insurance carrier and medical providers to assure continuity of care during transition times.
VNA’s insurance case managers communicate directly with insurance providers on patient progress and status and negotiate contracts for individual patient needs.
Our home care and hospice coordinators are available at several hospitals to assist in making a smooth transition from hospital to home. They assist the hospitals’ discharge planners by discussing and planning individual home care needs with families before the patient leaves the hospital.
VNA’s certified nurse aides (CNAs) access each patient’s care plan before starting care in the home and document their work at the end of each visit. Our nurses, therapists and medical social workers use laptop computers to access the patient’s care plan and document his or her care.
VNA offers expert, skilled and experienced caregivers
All VNA caregivers receive an in-depth orientation to VNA’s policies, procedures, philosophy, expectations and documentation to assure that they are focused on providing the highest quality of care. Orientation includes classroom training, co-visits with other experienced VNA caregivers, demonstrations of procedures, manager coaching and competency tests on a broad range of knowledge and skills.
VNA caregivers benefit from ongoing, mandatory education by clinical staff, assuring competency in current homecare treatment modalities before providing care to patients.
VNA’s community volunteers assist patients and families, both by providing a personable, social contact for patients and by offering occasional respite time for family caregivers.

The VNA Performance Improvement department has three, experienced, registered nurses, extensively trained in quality improvement processes and methods, to review and enhance our policies, procedures and educational materials to improve the care of all VNA patients. The VNA Continuing Quality Improvement (CQI) committee meets regularly to monitor care and improve policies and procedures to assure the highest quality of care.
The VNA Professional Advisory committee (PAC), consisting of community physicians, pharmacists, nurses, therapists, medical social workers and quality management personnel, meets bi-monthly to review and continuously improve VNA’s clinical policies and practices, ensuring consistency with the current standards of treatment for home health care.
The VNA Ethics committee, which includes a community representative and medical professionals, meets bi-monthly and as needed. Our goal is to optimize outcomes for patients; assure compliance with professional codes, legal requirements and the agency’s values; and balance risks and benefits for patients, caregivers and VNA in decisions that involve an ethical issue. The committee reviews and discusses cases, identifies alternative resolutions, gives guidance to managers and caregivers and provides education and training to staff.
