VNA Supports Independent Living
If you or a loved one is recovering from illness, surgery or injury, or need care for a chronic illness or disability, call on Visiting Nurse Association’s expert team of medical professionals. We will develop, with you, an individualized, comprehensive care plan to promote continuity of care for your – or a loved one’s – care and recovery at home.
Our registered nurses, therapists–physical, occupational and speech, social workers, certified nurse assistants and personal care providers collaborate with your physician to assure continuity of care, at home, as well as when hospital care is needed.
Our comprehensive team of caregivers is dedicated to clinical excellence, patient safety, continuity of care, expertise and quality.
VNA also provides continuity of care from hospital to home care by providing on-site coordinators at key hospitals. Coordinators work with the physician, the patient and the family to arrange for appropriate post-hospitalization services.
HOPE Program (Home Outreach Pharmaceuticals Education)
Through this grant-funded program, Visiting Nurse Association is able to provide vital supportive services to people who are HIV positive and homeless, or in transitional housing. The HOPE Program helps clients maintain independence in a supportive environment.
The grant is given to the VNA by Housing Opportunities for People with AIDS (HOPWA).
